Diese Pflegeanstalt auf dem Gelände des heutigen Schulzentrums (Kurfürst-Salentin-Gymnasium und St. Thomas Realschule plus mit Michaelskapelle) diente als öffentliche Anstalt zur Unterbringung von "unheilbaren und gefährlichen Irren" im Regierungsbezirk Koblenz. Sie wurde von 1835 - 1920 betrieben. Neben der Irrenbewahranstalt St. Thomas existierte in Andernach seit 1876 die Provinzial-Heil- und Pflegeanstalt (heute Rhein-Mosel-Fachklinik), welche neben der Pflege zunächst das Heilen der psychisch Kranken in den Vordergrund stellte. Während der NS-Zeit beteiligte diese sich an Zwangssterilisationen und und dem Massenmord im Rahmen des sog. "Euthanasie-Programms".
